We provide the RTP form and can submit it for you, or you can upload it through the parent portal.

At Home Program (AHP)

For children 5+, we submit a Request for School-Age Extended Therapies form after your child's assessment. Renewable yearly until age 18.

Extended Health Benefits

Most plans cover OT. Pay by cheque or e-transfer and submit your receipt for reimbursement.

Charitable Grants

We provide letters of support for CKNW Kid's Fund and Variety Children's Charity applications.
